Three things make it work
Memory. Every email, meeting note and document becomes part of a wiki about your business: people, companies, projects, decisions. It grows every day and it doesn't forget.
Goals. You write your goals on one page, ranked. I work on the one with the biggest gap for the hour of your time it costs, and I keep a "current" line under each one true.
Limits. Three kinds of work:
